WSUS Offline no longer installing important updates

I just rebuilt the client installer folder today

IT installed all selected updates but no critical updates.
I re-ran it multiple times, but it never installed critical updates

Same here. I did an update yesterday only to have a bunch of supposed "downloaded" elements fail. Then I rebooted the machine and reran WU - and it found the original 16 updates that it was barking about before I did the manual WSUSOffline update.

It rather looks like the catalog has issues. Remember, WSUSOU uses the catalog wsusscn2.cab and WUA for determining updates, exclusively. If the catalog is b0rked WSUSOU is, too.

There's a different WU issue I've heard of recently where the wonderful WUMT utility (Windows Update Mini Toot from "stupid user" [sic]) fails or crashes because MS has unwanted empty records in their WU database files. It could, maybe, have a side-effect on wsusscn2.cab/WSUSOU...

Hmm, they may really have borked the file on several load-balancing servers. Today my download run repeatedly downloaded the file claimings it's newer.
My older repository for XP and the newer for recent OS have files of the same size but their timestamp is different by four minutes: one 8:12 hours and the other 8:16 hours (hash differs as well). Judging from viewtopic.php?f=4&t=5612 there even may be a third one timestamped 8:17 hours

Currently I'd recommend you to revert wsusou to the April updates & wsusscn2.cab and wait for MS to fix wsusscn2.cab.

Please check if wsusscn2.cab is digitally signed (right-click on file, properties). If not delete the file + the corresponding checksum file \md\hashes-wsus.txt and restart download to get a new version. Verify it again for digital sign.
In my two repositories I have digitally signed files with two different last change dates but with same checksum:

I had the same problem with the wsusscn2.cab file when I ran 10.6.2 yesterday (13 May). Does WSUS Offline Update verify the digital signature in this file? When I ran WSUS Offline Update yesterday, it used the borked file and downloaded several URLs. Today, I deleted the cab file and the md subdirectory and this time got the file with the hashes that Denniss posted.

Here is what sigcheck returns for the borked file that was downloaded yesterday:
