Nothing went wrong here. You just must know that WSUS Offline Update and Windows Update are two different mechanisms for updating Windows. Updates installed by WSUS Offline Update don't show in the Windows list of installed updates.

About the space difference on your disks: The update installer saves existing system files by default. If a Service Pack is included in the updates, this would about come to that amount of space.
