@Aker: I checked the security permissions for both the user temp and the windows temp directory. The user for svchost/wuauserv is SYSTEM. SYSTEM has all but special permissions on both temp folders.
I took a little more care with updating my 3rd system. I verified that running UpdateInstaller without Update Sliverlight checked resulted with the same error as before and no updates being installed. I re-ran the UpdateInstaller with Update Silverlight checked and there was no error and updates were installed. Seems to be fairly solid confirmation that my problem was related to Silverlight in some way.
I found an old virtual machine that was 2012R2. This is basically a fresh install. The only 3rd party software installed was ClassicShell. I ran UpdateInstaller and it installed some updates, but eventually got stuck with the same error (required multiple runs). I re-ran the UpdateInstaller and confirmed that the error was still present and no updates were installed. However, this time I uninstalled Silverlight from Program and Features. Running UpdateInstaller again, I got no error and additional updates were installed.
So, for me at least, the problem was tied to Silverlight. I had to either uninstall it or batch it for the UpdateInstaller to function correctly.
