Installing the updates on XP

Have downloaded the XP package and installed to at least six XP systems. Each time, the computer says Nothing to do. But when the computer is connected to MS updates, there are updates to perform. Have changed out the wsusscn2.cab file, updated the wsusoffline package, and verified or not, the updates with the same issue. Nothing to do, but the October 2013 updates from MS gave me 14 updates, which the WSUS offline did not.

Any reason with possible suggestions on what to do would be greatly helpful
