Only security related updates are downloaded and installed. Being high priority does not automatically mean security related. In Windows 7, for example, you'll get recommended updates masked as ''important'' as an option (which is enabled by default). Other updates (like MRT or EEA Browserchoice) are blacklisted in WSUSOffline, as they make no sense when installing a system offline.
Once WSUSOU is complete installing the patches, following reboot, it is then as safe as it can get to go online and let Windows Update install the remaining patches. WSUSOU is not meant to replace Windows Update!
